From Theory to Practice: Lessons from My Clinical Psychology Internship.

This article was written by Quratulain Tahir, a B.S. Psychology student at Jinnah University for Women, who joined the Clinical Psychology Internship in Mental Health Care at the IQ Institute of Neuropsychiatry in July 2025. In this piece, she reflects on how supervision, role plays, and direct client exposure shaped her skills and helped her bridge the gap between theory and practice.

I joined this internship because I wanted real client exposure and the chance to observe how therapy sessions are actually conducted. While I had studied theories and techniques in class, I was eager to see them come to life in clinical practice. My goal was to understand what it truly means to sit across from a client, listen to their story, and respond in a way that is professional, supportive, and meaningful.

From the beginning, the supervision we received was structured, insightful, and practical. Our internship’s supervisor guided us step by step, ensuring we not only understood concepts but also learned how to apply them. The initial classes placed strong emphasis on roleplays, which proved extremely valuable. For example, if a client begins to cry, sometimes the most helpful response is to remain silent, give them space, and allow them to express their emotions until they feel more comfortable. On the other hand, if the client is a teenager who is unwilling to share anything and has attended only because of their parents, the therapist can explain how the session or therapy may actually benefit them. Such activities gave me a sense of the diverse clients we may encounter and taught me how to approach each one with the right mindset and techniques. They also highlighted the importance of empathy and adaptability, since every client brings their own unique challenges.

When it came to working with clients, I quickly realized that therapeutic interaction involves much more than simply asking questions. Every client is different: some take time to open up, while others may present significant communication challenges. At first, I focused mainly on gathering information. With supervision and practice, I began to appreciate the deeper elements of interaction such as validation, clarifying responses, and creating an atmosphere where clients feel heard rather than judged.

I also experienced a situation with a client where I was initially misled due to their manipulativeness. With my supervisor’s guidance, I was able to recognize this and then respectfully confront the client, which led them to provide more accurate information. This was a very valuable learning moment for me and gave me a skill I know will be essential in my future work.

A particularly valuable lesson was learning how to respond when clients are not fully open or when their accounts are inconsistent. Before this internship, I had not given much thought to such situations. Through guidance, I came to understand that respectful and thoughtful confrontation can be an essential therapeutic skill. This was a new realization for me, and I believe it will prove extremely beneficial in my future as a therapist.

Although I would not claim to have mastered advanced skills yet, this internship helped me build a strong foundation. I practiced validating clients, paraphrasing accurately, and using appropriate confrontation when needed. These may seem like basic techniques, but they form the core of effective therapeutic practice. I now feel more confident in developing them further.

Beyond skills, this experience also encouraged personal growth. I learned the importance of being patient, open-minded, and reflective. Supervision and feedback were crucial, and taking time to evaluate my strengths and areas for improvement after each interaction made a real difference.

For future interns, I would recommend coming in with curiosity and openness. Ask questions, seek feedback, and view every interaction as an opportunity to learn. Even small moments with clients can offer lessons that books cannot. Most importantly, view this internship as an opportunity something that, if embraced fully, will provide you with skills, experiences, and insights that will stay with you throughout your professional journey.

This program has been an invaluable step in my path as a psychology student. It gave me the exposure I was looking for, strengthened my foundation as a future therapist, and showed me how theory and practice meet in the service of helping others.
